Wheatbelt Population Health Services – Avon & Central Primary Health Service

Wheatbelt Population Health is a strong, diverse and high-performing health service that puts the health of country people first. Our priority action areas are:

  • Maternal and child health & development
  • Chronic disease prevention and self-management
  • Public health and communicable (infectious) disease control
  • Aboriginal health

The Population Health team is made up of four primary health services located in the Avon & Central, Eastern, Western and Southern districts of the Wheatbelt. A regional Wheatbelt Aboriginal Health Service and a regional Wheatbelt Public Health Unit are located in Northam.
